Most of you have now received your letter from Dr. Pobee notifying you of her decision to close her practice here at SCFH on October 31, 2024.
If you did not have an email on file or it is outdated, you will be receiving your letter in the mail over the next few business days. We have attached a copy of Dr. Pobee's letter for quicker accessibility.
We are offering a wait list for anyone who is currently rostered to Dr. Pobee, however, this comes with no timeline and no guarantee that a new physician will be joining SCFH and we highly recommend that you contact Health Care Connect. HCC is a program launched by the Ministry of Health and Long-Term Care to help Ontarians without a family health care provider find one and you can register quickly and easily by visiting their website Health Care Connect.
If you wish to be added to the waitlist, you can call the clinic at 519-925-0017 and by selecting option 4, you can leave a voicemail with your request or you can email scfhstaff@gmail.com. Please make sure to include your first and last name and your date of birth. If you are requesting on behalf of your family members, remember to include their information as well.
Due to the high volume of calls we will be experiencing, you will receive confirmation within 3-5 business days that your request has been received and that you have been added to the waitlist. This will come by phone or email.
