DAFHT Programs

Chronic Pain

The chronic pain program aims to provide education and strategies you can use to better manage your pain and improve your quality of life. Our program begins with an initial assessement with our Occupational Therapist and Clinical Pharmacist.

Diabetes

Living Well with Diabetes provides the education, tools, and on-going support needed for patients living with Type 2 Diabetes. Our team works together with Diabetes patients to help them manage their disease and maintain healthy living.

Foot Care/Chiropidy

A registered chiropodist works to prevent and treat various foot disorders that may develop as a long-term complication of Diabetes or other conditions.

Hypertension

Our nurse educators will assist you in developing a plan to help improve your blood pressure.

Mental Health

Our team of Mental Health Therapists support patients in enhancing their coping abilities, increasing problem-solving skills, and improving daily functioning.

Assistance with referral to community or regional resources may be provided.

Therapy groups are offered throughout the year for patients who may benefit from a group experience.

Nutrition

The dietitian will provide one-on-one counselling to patients regarding a variety of dietary based interventions.

Occupational Therapy

The goal of occupational therapy is to promote health and well-being by improving participation in everyday life.

Primary Care Pharmacist

A clinical pharmacist works directly with physicians to ensure patients’ best use of medications.

The pharmacist will help patients resolve any problems or concerns to ensure patients get the maximum benefit from their medications and discuss how medications are best taken or used.

Quitting Smoking & Vaping

Our team of trained Smoking Cessation Counsellors encourage, educate, and assist patients in creating a successful plan to quit smoking and vaping.

The program’s goal is to provide counselling sessions in a supportive way and have patients learn about the reasons they smoke and/or vape and gain tools and resources for their journey to success.

Stress management, coping strategies, barriers/triggers, and smoking/vaping cessation aids will be explored during the appointment.

Respiratory Therapy

The goal of the Respiratory Program is to provide assistance to patients with respiratory symptoms. This involves giving appropriate education, resources and symptom management tools based upon diagnosis. The Respiratory Therapist will perform a spirometry (breathing test) on patients with respiratory symptoms or smokers at risk for the investigation of COPD (Chronic Obstructive Pulmonary Disease) and Asthma.

Social Work

Social Workers provide short-term services to patients and support their social and mental health complexities.

Wound Care

A wound care nurse will complete an assessment of the wound and the individual. Identifying the wound cause, and treating the cause are the goals of best practice wound care.
